Childhood Disability Questionnaires

The CDP Childhood Disability Questionnaire is a valuable tool used to help judges better understand the nature and severity of our clients’ impairments.

The questionnaires ask how a child is functioning at home, in school, and in the community. We routinely request that a child’s providers — including doctors, therapists, and teachers — fill out the questionnaires. These professionals are in the best position to comment on how a child’s impairments affect his or her functioning. CDP will submit completed questionnaires to the Social Security Administration. The questionnaires are broken down by age group below.
